Electronic Records and Signature Disclosure
LCS Onsite, operated by Lansky Center Solutions Inc. Version 1.0 · Effective [DATE] · DRAFT — pending counsel review
Before you sign a document electronically through LCS Onsite, please read this disclosure. It explains your rights when you receive records and sign them electronically instead of on paper. You are agreeing to it for the document you were sent, by the person or company named as its sender.
Agreeing to do business electronically
By ticking the box and continuing, you agree that the sender may give you this document, any notices about it, and a copy of it once it is signed, in electronic form, and that your electronic signature (a drawn, typed or uploaded mark you adopt, or your approval) has the same effect as a handwritten signature on paper.
This consent applies only to the document you were sent and the notices about it. It does not cover other documents unless you agree again.
Your right to a paper copy
You may ask the sender for a paper copy of any document you are asked to sign electronically, before or after you sign it. There is no charge for a paper copy requested through LCS Onsite. To ask, reply to the email that brought you the document or write to the support address below with the document's reference number.
Withdrawing your consent
You may withdraw your consent to receive this document electronically at any time before you sign it by choosing Decline to sign on the signing page and saying that you want to receive it on paper instead. If you withdraw, the sender will need to give you the document another way, which may slow things down. Withdrawing does not undo a signature you already gave, and does not affect the legal effect of anything signed before you withdrew.
Keeping your contact details up to date
Notices about this document are sent to the email address (and, where you have agreed to texts, the phone number) the sender entered for you. If it changes before you sign, tell the sender, or use Reassign on the signing page when the sender allows it.

Your copy
